You accept that your device will be disposed of by OMSUK if you do not collect it within 15 days after either: * the date of the invoice for the repair * the date we notified you your device is ready for collection * the date we notified you to advise you that the device is beyond economical repair You accept that you will be charged £50 + VAT for secure disposal and recycling of your equipment.

CHARGES: Standard Evaluation = £76 + VAT Priority Evaluation = Additional £20 + VAT Insurance Evaluation= Additional £20 + VAT Engineer's time per hour - Residential - £76 + VAT Engineer's time per hour - Commercial - £92.50 + VAT TERMS & CONDITIONS: If your product is capable of storing software programs, data and other information, you should make periodic backup copies of the information contained on the products hard drive or other storage media to protect the contents and as a precaution against possible operational failures. Before you deliver your product for warranty service it is your responsibility to keep a separate backup copy of the contents, and disable any security passwords. IT IS POSSIBLE THAT THE CONTENTS OF YOUR HARD DRIVE WILL BE LOST OR REFORMATTED IN THE COURSE OF WARRANTY SERVICE. OMSUK LTD AND ITS AGENTS ARE NOT RESPONSIBLE FOR ANY DAMAGE TO OR LOSS OF PROGRAMS, DATA OR OTHER INFORMATION CONTAINED ON THE MEDIA OR ANY PART OF THE PRODUCT SERVICED. Your product will be returned to you configured as originally purchased, subject to applicable updates. You will be responsible for reinstalling all other software programs, data and passwords. Recovery and reinstallation of software programs and user data are not covered under this Limited Warranty. The client should always back up machines out of warranty. Also for machines in warranty, please note that faulty parts (inc. hard drives) become the property of OMSUK Ltd once replaced. Part of the diagnostics may include sending hardware/software information to Apple Inc. If we are unable to repair or diagnose the problem within our workshop, your device may be taken to a third party repairer or sent via courier or postal service to a third party repairer. OMSUK Ltd are not responsible for works carried out by any third party, including safety of your data. You should ensure your device is securely erased before booking it in for repair. If you are unable to do this, please make us aware so we can inform you of alternative options. All faulty parts become the property of OMSUK Ltd once replaced unless otherwise stated. Please sign below to indicate that you accept the information on this form is correct, you accept the terms stated above, and you agree that we may proceed with your repair (inc. evaluation/insurance report charges if required, please enquire). We reserve the right to dispose of any equipment and/or parts and accessories not collected by you within 15 days of the invoice date.
